Legal Question in Family Law in Georgia

I need to have my custody case moved from Illinois to Georgia due to our residency being in Georgia for 2 years what form would I use to file?

That's not done with a form,and generally it'snot something to even do, unless you're considering modification or contempt. And in either such case,the case will likely go very badly without a lawyer.
