My daughter is going into the military and she needs to give me full custody of my grandbaby. What forms do i need to fill out?

You didn't mention anything about the father, who also has rights. You may want to consider an adoption. Consult with a local attorney.

Charles Field's answer is frankly a bad one in my opinion. Adoption is inappropriate unless the mother permanently wants to give up the child.

I'd suggest a temporary guardianship in probate court. Be aware that the father may have a role in this.

But that is not enough. Additionally you need to see a lawyer, and have the lawyer prepare a will and power of attorney for yourdaughter. And whether you use the lawyerfor the guardianship or file pro se, talk to a lawyer first to know all the pros and cons.
