FIling for Separtation and divorce

I am married to a retired Military and we've been married for 9 years. I was diagnosed with Breast Cancer in 2004 and had surgery and chemo. I have yet to be placed on my husband military Id after all of this time. I've been abused emotionally and would like to leave before it gets any worse than it is. What I would like to know , what are the steps that I would need to take in order to get this process done. I know that I am not entitled to his retirement pay that he receives everymonth, but since I am his dependent and I do not have an ID, would he need to pay spouse support or anything on my medical bills that is outstanding? I was working when I was diagnosed with Breast Cancer and my company was bought out by another and moved to Texas. While I was being paid with my disability (long and short term) I did not receive my servrance pay because of the disability that I had received during my time off. I was told that If I had the military ID with the insurance that I would not have to pay as much as I did. I would like to know the steps to proceed.
