Legal Question in Family Law in Georgia

Where can I find minor name change forms for GA? The county that I reside in does not administer these forms and I have attempted to locate them from several places online without any success. The father and I were never married, and he did not sign the birth certificate so it really doesn't relate to custody. I just did not see any other heading to reference.

The reason forms are hard to find is that you can mess up badly with forms and most courts discourage you from wasting filing fees on an unsuccessful case. A case where the father is not on the birth certificate actually adds some complexity, so you do want a lawyer. The cost to do it right, with a lawyer, is usually reasonable.
