There are no forms. If you proceed without an attorney, you will be presumed to know the law and procedure and may suffer the consequences.

There are no "forms" for an answer. Whaat may make sense is to pay for a consultation with a lawyer, who can advise you on what steps you should do and how to do them pro se. The cost of that would be minimal. Call my office if we can assist with that.
