How do I format a letter to release my attorney and file my release if I am going to represent myself.

You say exactly what you just said.

Note that is extremely foolish to represent yourself in any case. Even lawyers hire lawyers to represent them in court cases. But if you want to go into a court unarmed, simply say what you just said in your post.
