First of all this is NOT a do it yourself project.
Legitimization is one step of what can be a complicated journey which ultimately will also will or may address things such as support, visitation, health insurance, college, tax deductions, custody and a parenting plan.
Do not attempt it without a good attorney, who will draft the needed documents (do NOT use "forms"), attend hearings, and work out the strategy that best addresses the upcoming issues.
Having the mother in agreement on those issues helps. The process will be far cheaper.
