legal age to decide to live with other parent

Can child live with other parent at age 14 even if other parent objects? where do you get forms for petition court to change custody and child support orders?

Unlike magistrate's court, where forms are available, you will need to file a modification in Superior Court. They do not provide forms. You should consult with a local attorney.

All changes of custody require a Judge. You can't just write a form. While a 14 year old can make an election, a Judge can, for cause, overrule it.
