Legal Question in Family Law in Georgia

I am living in Muscogee County, Ga. My daughter was born here July 9, 2011. At the time of her birth I was married, and my divorce came through July 11. As a result, I now need to change her last name, my last name, and add the birth father to her birth certificate as it is presently listed as "refused." I know that I need to go through the Superior Court here, but I do not know what forms I need to fill out. Is there any one that could help me find the forms? I can fill them out myself, I simply need to know which ones.

If I am understanding your question, you gave birth to a child not your ex-husband's. If that is the case, you have a huge legal problem. Unless you addressed paternity in the divorce, legally that child is your ex-husband's so you can't just add someone else to the birth certificate.

You CANNOT use a form. There is no form to clean up that serious a legal mess.

You will need to get a lawyer, see if the divorce can be reopened to determine paternity, and then. after DNA tests and hearings, you may be able to later correct the birth certificate.

You have a highly complex situation and no form exists for it. If you had used a lawyer in your divorce you could have addressed all this then.
