I was married on a cruise in Jamaica (2001) and never registered in the US, changed my name or filed taxes as married. Will I need to file for divorce or would an annulment be possible?

There is no such thing as registering a foreign marriage in the U.S. If your marriage was lawfully done, then you arre married, and if you want to separate you need a divorce lawyer and a divorce. Annulments only happen as to void marriages.

There is no requirement to take someone's name when you marry.

You may also want to see a tax attorney as you could be prosecuted for 9 years of tax fraud unless you filed as married or married filing separately. You need to file 9 years of amended state and federal tax returns. You probably will owe some taxes and penalties but fixing this could avoid criminal charges at some future point.
