modification of visitation
my husband has sole physical custody of his twin 11 year old boys. the children's biological mother has been arrested twice this year for child abandonment, she floats in and out of their lives. the psychologist that the children see recommends very strongly that we change her visitation rights to include supervised only visitation because of ''lack of contact, inaccessibility, erratic behavior, relocation to a new home with yet another new boyfriend, and legal involvement''-so i basically need to petition the court for modification of visitation rights. the only petition i could find was a petition for change of custody:claim for modification of visitation rights. but i'm only looking to modify viitation-what is the title to the petition i need to serve this purpose? i've typed up the petitions i've needed without the assistance of an attorney for the custody papers and for contempt of court, so i know my way around a courtroom. i cannot afford an attorney- but all i need is the correct petition. please see if you can help me in this most critical issue for ''my'' boys.

I would strongly urge you to NOT do this yourself. If you must, you would be best served paying a lawyer to at least ghostwrite it for you.
A couple counties in Georgia are a bit pro se friendly (Fulton and Deklab) in Family Law and both those courts offer some forms (no other Georgia courts do), so you might visit them for guidance. But even they recommend you review your papers, before filing, with counsel.
