Best way to obtain custody of a child, when you are not a biological parent

I have raised a cousin's child since birth the child is 7 years old now. I have no idea where the birth mom is located and never knew who the dad was. Birth mom asked me to raise the child during pregnancy, but we didnt' sign any legal documents. Last I heard the birth mom whom is in another state, was a drug addict along with frequent visits to mental institutes. I want to make things legal to avoid any problems in the future.
