Power of Attorney

I have been my Grandmother's Power of Attorney for 14 years. She was diagnosed with dementia in Feb. of this year. My sister obtained Power of attorney two months ago, then put my Mom (Grandmother) into a nursing Home. Mom had dementia at that time. I wish to remove her from that facility, and bring her from Douglasville, Ga. to Owensville, IN. to live with me. How do I accomplish this legally? I need to make this move quickly to restore her physical health. She is Not being well taken care of! (I have photos) Please respond quickly.
