Forms from who? If a lawyer has drafted an agreement for you, ask him/her. No one here has the "forms" to answer your question. If you get generic forms from the internet, you take the risk they are worthless (and often they are).

Forms for a prenuptial? That's an awful idea. That is a pretty good way to end up with useless paper courts probably won't enforce.

What you want to do is have an attorney draft one that specifically fits your situation, so it will have a better chance of being correctly written and enforceable.
