Why was the proposed parenting plan and the actual parenting plan totally different, and my signature was forged on the actual, what are my legal options

No one on a website knows what happened in your case, or why. If a document was forged, your first step should have been to immediately contact a lawyer to take action. Doing nothing in the case, or delaying, indicates it is not that important or it wasn't a forgery. People who find out their name has been forged to a document filed in court generally take action in court within a day or so.
