Legal Question in Medical Leave in Georgia
Family leave act
I am told that I can apply for family leave act as my father is having surgery in California and I live in another state. I am told that I must have my father take a form WH 380 to his doctor and then I will submit it to my HR office. Am I correct and is this the correct form.

Form WH-380 is the form that health care providers must complete to certify that their patient has a serious health condition and that the person requires assistance from a family member. You can find these forms on the website for the U.S. Dept. of Labor.
