Legal seperation

What does legal seperation cover? Are there forms to facilitate the process? Is it reversable?

What would be the circumstances for filing for a legal seperation rather than a divorce?

A legal separation, or "separate maintenance" is exactly like a divorce in that issues of division of property, alimony, custody, child support are addressed. The only difference is that no divorce is granted. You still need to file a petition. I don't know if there are forms available; they certainly aren't available at the courthouse, whereas forms for, e.g., small claims court are available. The action can always be dismissed, or, the parties can simply reconcile and get back together.
