how can I get someone to pay back a loan

I lent someone money in august 2003 for a deposit 1408.00, had them sign a promissory note that I downloaded off of the internet. I which they specified the repayment plan, signed and returned to me. I, to date, have only received two payments of 40 dollars. I am ready to collect on my money, for lack of consideration with contacting me about reasons for missed payments. Where do I begin? And is the promissory note any good?

As long as the promissory note contains a "promise to pay" it should be perfectly valid. Just to be safe, I would call around and find an attorney in your area who offers no-charge initial consultations and who can take a quick look at it and see if all is well. Then, you are ready to head to the Small Claims court in the county of residence of whoever owes the money. They will have forms and instructions for you.
