Can a medical office charge for completion of FMLA forms or can no fee be charged because it is a state required form?

You provide no facts, but FMLA forms are not a "state required form" to the extent you think there is an obligation for a provider to do the work and complete the forms, or that the provider is somehow directed by the state to do the work. They are forms completed by a provider, based on a request from an employee seeking approved leave. Thus, the provider is providing a service based on a request from a patient, and for the benefit of the patient. Like other services they provide, there is usually a charge and that charge is usually paid by the patient (or the patient's insurer). If you are asking about some forms other than the forms completed to request family leave, or you have a specific reason to believe the doctor's office is somehow obligated to provide free services to the employee, re-post the details.
