I'm on a rent to own lease with my ex-fiance, he moved out a year ago; how do I get his name off the lease?

No on knows the terms of the documents. Your ex likely still has responsibilities or rights under the contract. Discuss with the landlord, or if it worth it, see a lawyer. Landlords generally have no incentive to let someone off the hook for payments.
