can you legally refuse to make an existing patient an appt with a large old outstanding balance?

This appears to be in the incorrect category. Who is "you?" Lawyers have clients, not patients. Are you trying to ask if doctors can refuse to see patients who owe them money? Sure they can, if in private practice. Why would they be expected to extend more credit to a patient who does not pay?

I am confused. You asked this under legal ethics. Lawyers do not have patients. They have clients. But yes, a lawyer, just like a doctor or anyone else, can refuse to do business with someone who has not paid him.
