knife injury due to walmart negligence
I bought a paring knife at walmart. It came out of the package, I handed it to the cashier so she would not get cut. She stuck the knife in the bag without rapping it. When I was loading my grocery into my truck the knife went into my arm 3 inches. Walmart said they would take care of it. Then the next day they have to do further inverstigation before they will pay for my doctor. They want me to sign a medical and report release to finish stage one. What should I do get an attorney or sign the release and see if they are going to do right by me?

I cannot advise you on whether to sign the release without looking at it. However, if it is simply a consent to obtain your medical records, it might be OK, but if it was any language releasing any claims, definitely don't sign it. You already made a bad decision. Never talk to a retailer in such a case. What you have already said and done could have compromised a claim you have, if you have one.
Always see a lawyer first. Obviously don't sign anything or provide any records without that step.
I can't say that you do or don't have a claim that a lawyer can help with without a lot more information, but talking to the business first is always the wrong step. The only communication should have been to have documented the injury at the scene.
