Home ownership

My mother who is an 83 year old widow wants to transfer the deed to her home to me (her only son) in order to protect the asset in case she someday has to go into a nursing home. What is the typical procedure for this.

Real estate is transferred by a deed, typically either via warranty deed or quit claim deed. Note that such transfer will not also transfer any mortgage and may even be in violation of such mortgage.
