Property as Collateral

I just found out that my elderly grandmother signed her house over as collateral for my nephew to get out of jail. She is not well and has been hospitalized with medical problems in the past. My sister has power of attorney over her and did not know this had been done. What can we do to rectify this matter, just in case he defaults?

Probably nothing, unless you can show that your grandmother was not capable if executing such guarantee, which would probably be very difficult.
