What is the process to remove a person's name from a deed to a house?

You can't remove a name from a deed. However, if A and B own the property, they can sign a deed putting the property in A's name alone. Consult with a local attorney.

Transfer the property via a deed. However, if there exists a mortgage on the property at issue, such transfer may not be permitted.
