Legal Question in Tax Law in Georgia

For the state of Georgia, am I allowed to get financial reporting documents for a 501c3 (the organization is a neighborhood association of which I am a member) ?

I am particularly interested in a list of donations they received for a specific time period. Is there an official IRS form I must fill out ?

You can't get it from the IRS.

Whether you can get it from the association (and how) depends on many facts that you have chosen not to give us Many of those answers are in paperwork that you have and no one here has seen.
