Name change on a property deed
My father passed away in June and my mother needs to change the name on the deed to the house to her name. What is the form that she will need to have when she goes to Superior Court to file?

You do not give enough information. Was there a will? (if not, all heirs must agree.)
How was the house titled? (Father's name alone, tenants in common w/mother, joint tenants w/mother)

It is not just a form. In most cases a lwyer will need to help you probate the estate and can draft the needed documents.
