Legal Question in Workers Comp in Georgia

Workmans comp

I hurt my back in Jan.The doctor sent me back to work before I was able.I ended up hurting my shoulder when I went back to the doctor for my back .I asked him what was wrong with my shoulder it hurt terribly.He replied arthritis I told him I heard a pop,he insisted I see a ruematologist.I did 12 weeks later a mri was done and found it was a rotorcuff tear and requires surgery.I made a claim 4 weeks ago and it was denied because I did not fill out an accident report and could not (because of all the drugs ive been on for my back) remember exactly what day it happened.Is this worth fighting over?The doctor says I need this surgery or I could further damage my shoulder.I do have insurance but I am afraid they wont pay because it was work related.Please can you give me some insight on where I stand.
