amending a death certificate

I need to amend my fathers death certificate. He was married to my mother, although seperated there was no divorce filed. He then married another lady in the state of Idaho, 2 months prior to his death. This "other" woman was listed on his death certificate as surviving spouse, even though his marraige to my mom was never disolved. The "other" woman was also the informant for this death certificate. My grandmother passed away ther day before my dad (his mom) and left her pension to her kids and if they were alive, and if not the $ was to go to thier spouse. I need to amend his death certificate to my mothers name.

Re: amending a death certificate

Contact the office of the State Medical Examiner in writing. Explain the situation and include your proof, which is probably a copy of the first marriage certificate.

Are you quite sure he didn't divorce her in another county or state and you didn't know it? That happens fairly frequently.
