second motion for reconsideration
Can I get a sample of how to file a second motion for reconsideration as well as the motion for leave to file the motion?

Are you planning on filing a second motion to reconsider on the same issue where the first motion was denied? That's not necessarily a good idea. If you feel the court was wrong in denying your motion to reconsider, bring an appeal.
You did not say what the nature of your claim is, what the issue is, etc. That information is necessary if someone wanted to try to provide you with a sample motion.
