How do I fill out a bankruptcy cover sheet, and do I have to fill one out for every creditor?

No offense to you, BUT, this is a rather simple thing to do. It is evident that you are "pro se". Are you SURE that you do not want a lawyer to represent you??? Why don't you contact your local bar association and ask for help before you botch the whole thing.
