If asked, does a custumer service rep. for a fortune 500 company need to provide the name and a mailing address for the president and CEO of that company? If it is required to give out that information, what kind of penalties could that company face if the information was not provided and/or false information was provided?

No, not as a general rule. Assuming the company is publicly listed the information is readily available.
