I am a solo MDSC.
what form should I have at the yr end showing the meeting and minutes?

There are several formalities which are required for your corporation. It would be best for you to contact an attorney, since if you fail to fulfill these requirements in a timely and accurate manner, you could be personally liable for the actions and debts of your corporation.
