Liability form(s) for protection

Dear Legal Advocate;

I am starting my own body piercing business, and want to have my patrons sign a form before rendering services. Where can I find a general 'liability' form, that I may modify to fit my needs.

Most any stationary store that sell legal forms should have what you want... BUT I would spend the money and have a lawyer draft a special release form that fits your business and the fact and circumstances that surround your rather risky business.
