Legal Question in Business Law in Illinois
I currently have a Sole Prop - DBA in Chicago, IL. I will probably be hiring at least one employee in the next couple months.
My questions is: What do I need to do to convert to an LLC and get an EIN? Do I need to "dissolve" the Sole Prop/DBA? Please point me in the right direction.
ALSO: I am opening an office to do high-end smart phone repair by appointment (not store front). What business license do I need in the State of Illinois/City of Chicago?

You need to file the appropriate paperwork downstate and also develop articles of organization. If you are going to be selling parts, you will need to register for sales tax purposes.
You need only obtain a business license in Chicago.
As a Franchise Attorney I can say this about your DBA. You don't need to convert to another entity form, like an LLC or corporation, to get an EIN. You just apply for an EIN under your DBA. That would be the easiest way. Make sure to take out a good insurance policy also so your personal assets are protected. Consult with a good business or franchise attorney in your area for specific advice.
