no attorney, recived an arbitration award. need a form or format to sumit it to the court for a judgment. no form listed online for our county.

Generally, you need to file a complaint alleging the rendition of the award with a copy of the award attached. Then you need to have a summons issued and served on your opposing parties. Thereafter, the case proceeds as an ordinary civil case, though there are special rules that apply.

Forms are available at a law library. Of course, there's also a process associated with using the forms, so you have to read up on that as well, or hire someone who already knows how to do it.
