It is hard to tell from your question if you are a car dealer, or a car dealer customer. Punctuation is helpful. I will guess you mean you are a car dealer and you want to ensure a customer does not sue you. I am not aware of a "standard form," but an attorney could assist you in preparing a document which would protect your interests.
