Paid and billed

On a radiology test, the company accepts my insurance. I paid the co-pay and the insurance company paid the negociated amount. I was told by insurance I could be billed $0.00 by the company. The company has reversed the insurance ajustment and says I owe the balance. Is this legal? I'm I responsible for any more $$$?

What was the reason the Ins. Co. Denied the claim?
