Legal Question in Credit and Debt Law in Illinois

How do I put a levy on a judgment debtor's bank account? Is there a special form to fill out, do I have a sheriff serve the debtor bank account and seize the funds? If this a fact where may I obtain a bank levy form for southern illinois.

Do you also know where I may obtain the following, Writ of Execution, and Proof of Service forms?

There are forms and procedures for what are called "supplemental proceedings" to assist judgment creditors to obtain "satisfaction of judgment". These include "Citation to Discover Assets" ("CDA"s), "garnishments", etc. Depending on the information you have you can obtain a court order requiring the bank to provide specifics, put a hold on accounts, and ultimately obtain a "turnover" order to get assets "turned over" to you The Clerk of the Court where you obtained the judgment should have all of the forms you need, and if they are willing to help you with the procedure, that would be "above and beyond" except they try to help sometimes because without it court procedures can get gummed up. If and when you get "satisfaction" there is even a form that clears the judgment so that the record reflects there has been "satisfaction of judgment" and you would be required to provide the judgment debtor and/or third party with that kind of evidence.
