Written Appearance for CC Debt

I was just served a Summons this morning by a financial institution regarding an old credit card debt. I have to file a written appearance on Monday (it's Friday today). Is there a form for my written appearance? Or can someone just tell me what I need to include in it?

The circuit clerk keeps appearance forms on file. Just go to the clerk's office and ask them for an appearance form. Then you file it with the clerk and pay the cost asociated with the appearance.
