Final Disposition of a Case
After completing probation in a felony case in Illinois, is there a form I should receive as proof of my successful completion ? I need a form that states the charges, the sentence, and the final disposition.

In Cook County you would order a "certified copy of disposition" from the Clerk of the Circuit Court in the courthouse where your case was tried. I don't know about other counties, but I imagine it would be similar.
