what is the proper procedure for a legal name change in the state of illinois and do you have to change your original birth certifate afterward?

Here is the name change law for the State of Ill. I would start at the circuit court house. Most should have a standard procedure and petition forms for name change.

Here is part of the Ill law on this issue.

Illinois Statutes 735 IL CS 5/21-101 Proceedings; parties. If any person who is a resident of this State and has resided in this State for 6 months desires to change his or her name and to assume another name by which to be afterwards called and known, the person may file a petition in the circuit court of the county wherein he or she resides praying for that relief.

A change of name in Illinois legally begins with a petition which is filed

and then notice is given to all interested parties. The case must also be publised in a legal newspaper for an adult so that creditors will have constructive notice that one is changing his/her name. Will be
