If an agency sends me household help that has a broken ankle and they are able and willing to use my stairway am I responsible for them if they fall on the stairs in my house?

It will depend on the circumstances surrounding the fall. If there is something wrong with the stairway then you probably will be responsible. You should check the building code to find out what safety measures are required for interior/exterior stairways. To avoid the problem altogether, you may want to refuse the help.
