I've asked my lawyer to withdraw from my case, but he refuses, how do I get his appearance withdrawn from my case?

You can file a motion with the court saying you have discharged the lawyer and ask leave to file your pro se appearance. He or she is then off the case (unless the judge disallows your motion).
