my big boy
i have a 10 year old son whom i've raised since the age of 13 month. his biological father never bothered with him. i'd like to adopt my son and i want to know what form would be appropriate to present him(biological father) with; to give up his parental rights?

I need to know more. Who are you, and what is your relationship biologically and legally with the child and his mother? Getting someone completely removed from a child's life is not a matter of the right form. There are constitutional issues involved, and thus court hearings and so on.
