Child abandonment

My fiance has two kids from her previous marriage. Her Ex-husband has been out of the picture since 2000 until now. Is there any limit to the time that a parent has been out of a child life to revoke their visitation rights. Parent only had supervised visitation every other weekend for 5hrs. Also no child support has been paid since 2000 either.
