It depends on the context, I am assuming you were served written discovery asking for a list of witnesses. So you need to provide basic contact information (telephone number and address) for all of the "lay" witnesses and give a brief explanation of what they will testify about.

Example: John Doe, (xxx) xxx-xxxx, 1234 First St. Chicago, IL 99999. Will testify concerning the child's behavior at day care.

If the witnesses are professionals (therapists, etc) then the rules are somewhat different. Give us a call if you want to talk about the details.
