If a judge makes a decision on a case without the entire knowledge of the childs needs how can it be changed?

depends. If the missing information is really crucial (like the residential parent beats the child every day) then it can be reheard tomorrow. If its more along the lines of information showing the residential parent is not responsible, or why the non custodial parent would be better, you have to wait two years from the date of the custody order. There are exceptions and caveats of course.
