How do I go about getting legal forms to file a petition for joint custody & visition for my daughter.

Law library at local Courthouse (Joliet?)

Look for IICLE on Family Law. I think Volume 2 or West's Illinois practice of Family law Practice Book

plenty of forms

Be careful though. This is a tricky area of law. You could end up making things worse.
