Legal Question in Family Law in Illinois

i have not had contact with my childrens father in years he lives in az and i live in il. my son talked to his father thru his grandmother and made plains to visit him for the rest of the summer,the day my son left on the flight my son said he wants to goto school there. his father sent my daughter who did not want to visit him an email with a list of things he wanted like ss card birth certificate and also wanted me to sign my rights away.(we have had 50/50 custity by the court scence 1998,but he never would see the kids). my daughter set email to her father for me asking what school will he goto so i can sign a transfer form at the school here in il. i never got an answer. i learned by the school here that she aready sent the info because she has my signiture on the form from az that i and the father both live together in az. he and i dont live together he is married to an other woman for 10 years now. what do i do about them fordging my name and also they will not give my info to contact my son. he has not called or been on his myspace. there father also called dcfs on me because my daugther is having a baby with an older man. i dont know what to do?
